Training and Workshop Registration

Registration:

Complete the Registration Form attached at the bottom of this page. Send registration to fcarroll (at) west.net or by mail to : 540 Alisal Rd. Ste. 1, Solvang, CA 93463 USA. or by fax 805-693-9180. The registration fee includes materials, refreshments, and transportation from/to Santa Barbara airport if needed.

A deposit of $300, respectively, is necessary to reserve your space. You may pay deposits or the full fee with PayPal that is set up on this website. Make checks or money order in US dollars payable to Felicia Carroll, LMFT. Registrations are not complete without a deposit.

Fee Reduction:

Only a few partial fee reductions are available. Please use the Fee Reduction Form attached at the bottom of this page and email it to fcarroll (at) west.net. Fee reductions are granted on the basis of need and in order of date of application. Persons granted a reduced fee will be asked to assist Felicia Carroll with organizational tasks.

Cancellation:

All cancellations and requests for refund of fees paid must be in writing. A full refund, less a $75 processing fee, will be made for written requests by July 1, 2013 for the Summer Intensive Training. A 50% refund will be granted for request received up to July 15, 2013. No refunds will be issued for a cancellation after these dates.

For workshop cancellations, refunds will be granted up to 2 weeks prior to the workshop date minus a $75 processing fee.
