Registration:

Complete the Registration Form. Send registration to Felicia@feliciacarroll.com or by mail to : 540 Alisal Rd. Ste. 1, Solvang, CA 93463 USA. or by fax 805-693-9180. The registration fee includes breakfast, lunch, refreshments, and transportation from/to Santa Barbara airport.

A deposit of $500/$300 is necessary to reserve your space. You may pay deposits or the full fee with PayPal that is set up on this website. Make checks or money orders in US dollars payable to Felicia Carroll, LMFT. Registrations that are not complete without a deposit prior to June 1 and applications after July 1 must be accompanied by the full fee.

Fee Reduction:

Only a few partial fee reductions are available. Please use the Fee Reduction Form attached below and email it to Felicia@feliciacarroll.com. DEADLINE FOR FEE REDUCTION REQUESTS IS JUNE 15, 2008. Fee reductions are granted on the basis of need and in order of date of application. Persons granted a reduced fee will be asked to assist Felicia Carroll with organizational tasks.

Cancellation:

If you need to cancel before the balance is paid, your deposit will be refunded, less a $50 processing fee. If you have paid the full fee, a full refund will be made for requests received by July 1. A 50% refund (of the full fee) will be granted for requests received up to July 15 and no refunds will be issued for a cancellation after July 15.
